Oral Cancer Screening Consent

The Oral Cancer Screening is a diagnostic screening done by our practice to screen for oral cancers.  Some insurance providers cover this test, others do not.  If you are interested in the exam, please submit the form above and we will check with your insurance company to see if it is a covered service.
